Grant care providers immediate access to your test results and symptom records by providing them your Passport ID, a unique identification code received upon completion of an ImPACT®, ImPACT Pediatric®, ImPACT Quick Test®, or Cognitive Impairment Screener™ (CIS).ImPACT Passport includes the ability to:• Record ImPACT Passport IDs• Locate ImPACT-trained concussion care providers• Record and upload symptoms to your profile• Grant your healthcare provider access to your test results and symptom records• Generate a Completion Confirmation certificate for any assessment you’ve taken
